information provided by me in support of
my application for employment is
true and correct to the best of my
knowledge. I understand that
misrepresentations or omissions may be
cause for rejection, or may be cause for
subsequent dismissal, if I am employed.
In
making this application for employment ,
it is understood that an investigative
report may be made whereby such
information is obtained through personal
interviews with third parties, such as
past employers, business associates,
financial sources, friends, neighbors,
or others with whom you are acquainted.
This inquiry included information as to
your character, general reputation,
personal characteristics, and mode of
living, whichever may be applicable. You
have a right to make a written request
within a reasonable period of time for a
disclosure of parties contacted. I
HEREBY AUTHORIZE SUCH INVESTIGATION.
I
hereby authorize any former employer,
person, firm, corporation, or government
agency, to answer any and all questions,
and to release or provide any
information within their knowledge or
records, and I agree to hold any or all
of them blameless and free of any
liability for releasing any truthful
information that is within their
knowledge or records.
This
company is hereby authorized to release
to any other firm or person with whom I
may seek employment, any and all
information concerning my employment or
application.
If
given employment, it is further
understood that I am to be on probation
or trial for a period of 90 working
days. If during this period it is
discovered that I am not suited to the
work assigned me, I agree to the
friendly termination of my
employment by the company.
I
also understand that after the 90 days
probationary period, my employment is
for no fixed term, terminable by the
employer at any time for any reason.
This understanding cannot be changed
except by an authorized company
official.
In
consideration of my employment and
compensation to be paid to me by this
company, I do hereby agree to disclose
and assign to said company, it's
successors its assigns or appointees,
all inventions and improvements or
copyrightable material made by me
reasonably pertaining to the business of
said company during the entire period of
my employment, regardless of the duties
to which assigned; to file applications
for letters of patent for said
inventions and improvements in any
country designated by said company, it's
successors and assigns or appointees; to
assign said applications and any letters
patent issuing there-from to said
company, its successors and assigns or
appointees; and to perform all lawful
acts necessary to secure to said
company, it's successors and assigns or
appointees, any of said letters patent,
it being understood that said company
shall pay all expenses incurred in the
prosecution of said applications and
obtaining said patents.
This
application is current for only (60
days). At the conclusion of this time,
if I have not heard from the employer
and still wish to be considered for
employment, it will be necessary for me
to fill out a new application.
Drug
